Excellent care

Do not send your loved ones here! My grandpa was recommended here for his physical therapy. Staff were rude and unwilling to help. They left him unsupervised during the night, they did not go in to check up on him. Very unprofessional place. Don’t take your loved ones here, you will regret it.

My family and my self are so very disappointed in this facility!!! Staff is so rude n disrespectful and not professional not all but most. We took our loved one for physical therapy which we were recommended to stay at the facility for his care just to be upset all day and mad. Please do not take your loved ones here u will regret it!!

We truly loved the facility and the wonderful people (Melissa, Cindi and others) we met during my mom’s stay. By the time she left, the staff felt like family, and saying goodbye was genuinely hard. The therapy team (Phong, Blake and others), were incredibly patient, encouraging, and dedicated to helping my mom regain her strength and mobility. The nursing staff (Monica, Danielle and others) went above and beyond, showing her so much love, kindness, and compassion. They encouraged her, celebrated her progress, and treated her as though she were their own grandmother. We also appreciated the open communication and the relationships we built along the way. Like any facility, there were areas that could use improvement. At times, staffing seemed stretched thin, and the food was not always the best. However, those things did not overshadow the wonderful people who cared for my mom and made such a difference in her recovery. I really enjoy staying at North Houston transitional Care! The food is one of my favorite things about being here . The staff is very helpful and makes sure you are taken care of , and I appreciate the effort that goes into making my experience comfortable. Emely was so pleasant and supportive during my care plan meeting with her amongst other staff.

I have a good experience at NHTC, rehab program is Great and Phong Nguyen the physical therapist is wonderful

My mother was at NH from August 12 until she died on Sept 16. A few days over a month. I selected this location because it seemed like a nice place. It was clean during the walk through, the shared rooms are well designed and feel like a 2/1 room mate split. I loved that. The location is also fewer than 5 minutes from my job and less than 15 from.my home. Mom was here in hospice care with Gentiva. I was able to visit as often as I wanted and stay as long as I liked since they have 24 hour visiting hours and will even provide a cot if requested. Some staff members genuinely care about the patients and it showed. Some standouts were: Michelle, Kevin, Monica, Tiffany, Latasha/LaTanya (apologies, we met once when I stayed the night so her name is a bit hazy), Shantelle and Angela. There were multiple issues though. Two individuals (i dont think they were nurses, but idk if they were CNAs or patient care or what their actual positions were) who don’t belong in health care. I do know their names but have chosen not to state them here. After I inquired why my mother was lying on the floor while her bedding was being changed, 1 of them commented “well take your mother home then” when she thought I was out of earshot. She accompanied this comment with clapping her hand against the desk while the other young lady laughed. When I confronted them, I was told “we weren’t talking about your mother, we were discussing a different patient.” Either way I was offended since they were talking about somebody’s somebody and I told them so plus filed a formal grievance. This happened on Saturday August 22. Supposedly the grievance was faxed over to the administrator of the facility and the DON however, no one ever contacted me about it and it was never addressed. Multiple times I had to get Gentiva involved about my mother’s medication. My mom had advanced vascular dementia and a broken dislocated femur which is why she was in hospice. She had been there for a week and was still being given half a norco (1/2 of 5mg/325 hydrocodone/Tylenol) when I inquired about her hospice “Comfort meds” I was met with “she’s hospice?” I had to put a sign above her bed stating HOSPICE PATIENT. I repeatedly had to tell nursing staff when my mother was in pain simply by looking at her body language. Often I was told “she has not said anything about being in pain” as though they were genuinely expecting a woman with dementia, openly hallucinating, and talking to stuffed animals to say “my leg hurts.” I who have never taken an anatomy class had to explain what happens when you try to straighten up a person who has a broken/dislocated femur and the amount of pain it causes. I entered the room to find a worker spooning food into my mother’s mouth which probably wouldn’t have been ao bad if she was awake. She was not! When I commented about her not even being conscious he asked “can I take the food out of her mouth? I figured if I put some in, she’d wake up” then I watched as he used the spoon to scrape food out of her mouth. They place mats on the floor around a patient’s bed and do not count it as “falling out of the bed” if the patient lands on the mat. Neither do they report the “non fall” to Gentiva/hospice. I argued mats are not fall prevention, merely “soften” the fall that has already happened. I think NH has some really good people working there and it is better than some nursing homes I’ve visited/experienced, but there are a plethora of things that can be improved most of all their administration since any business is only as good as the people in charge. This place felt like all worker bees but no one was at the helm.

Very disappointing experience. Communication was terrible, staff were often rude and dismissive, and the overall level of care was not what we expected. Our family had multiple concerns that were not handled appropriately. I would not recommend this facility for a loved one.

Absolutely HORRIBLE experience. I would not recommend this facility to anyone looking for quality, compassionate care for a loved one. My mother has stayed at this facility twice, and both experiences were extremely disappointing. On multiple occasions, my mother was left sitting in urine and feces. Both stays were also followed by UTIs. Getting anyone on the phone was incredibly difficult, and when we did speak with staff, we frequently encountered rude and dismissive attitudes when we were simply trying to advocate for my mother’s care. Our discharge experience was especially upsetting. Marisol, the discharge coordinator, handled my mother’s discharge extremely poorly. In my experience, she was dismissive, made hurtful comments, and attempted to place blame on my mother rather than taking responsibility for properly coordinating her discharge. This was an already difficult situation for our family, and the way it was handled made it significantly worse. We also had serious concerns regarding billing. Even our insurance representative expressed concern about the amount the facility attempted to bill my mother, who is a Medicare patient. Our interactions with management and the billing coordinator were equally frustrating and disappointing. When families place a parent or loved one in a facility like this, they are trusting the staff to provide basic dignity, proper care, and communication. Based on our experiences here, that trust was repeatedly broken. Please think very carefully before sending someone you love here.

Staff is amazing in every area. My nurses and the food is pretty good. It’s edible. It really is and I was real nervous about PT and my first day was a wonderful experience and every day since has been awesome they push me but not too hard. they encourage and they’re so friendly and they know what their hint doing and I am so happy to be able to get better. I had had some bad experiences with my husband, and I was very nervous when I came here within A few hours, I have a very Positive feeling. Rest assure if you’re having to come here for your getting better or a loved one this is the best choice

I have had a very positive experience at north houston transitional care. From the moment I arrived the staff has made me feel welcomed,comfortable and cared for.

I’ve enjoyed my stay here , the food is great , my room is always clean & they always seem to be very attentive to my daily needs.The staff is very friendly as well.

Chris and Caroline are great trainers. They have given me confidence, comfort and reassurance. I’m learning to be self motivated without any fear. Thanks to them I have come a long ways. I will continue my journey. Linda Lozano

Therapy here is very good, Phong and Ben are very nice!

My 102 year old Mom lives here, and we could not be happier with it. The staff is exceptionally friendly and thorough with her care. She has social interaction with other resident every day, and she’s always happy when we go to visit. She has her own suite, including a living room, bathroom, bedroom and kitchenette, so she can be surrounded by her own things. Rates are quite reasonable, but I can’t recommend a better place at any cost.

Don’t, Don’t send your loved one to this facility. We had a horrible experience at this place. My husband had to lay in his own poop for up to one hour before anyone came to change him. He would request to be taken to the bathroom only to have the staff take their time in taking him causing him to soil his pants. I ended up having to change the linen on his bed because they were always too busy to do it. The nurses station was always empty and you could never find a nurse for assistance. The hallway smelled of urine. Since then, he ended up in a different rehab facility and it was 100% better than North Houston Interventional. Sad sad sad!!!

I can’t say enough good things about the therapy staff at North Houston transitional. I felt they were truly interested in my mom‘s progress and wanted to help her do well. They were patient and kind and I want to give a special shout out to Caroline in OT and Dan in PT. The gym where they work is fantastic, fully equipped, and very helpful

I have enjoyed my time here. The nurses are well appreciated. The food selections are all tasty as well as my room is always clean and well equipped.

I have had a very good experience with this facility. All staff members are kind, respectful and professional. The nurses and caregivers are attentive and patient with me. I’m overly grateful for the care i receive on a daily basis.

I viewed the ratings before I chose the facility, they were very high and I can see why . The PT staff has been very supportive and professional with encouragement at every level. The Nursing staff has been extremely understanding considering my situation of not being able to walk. The nutritional staff is constantly monitoring your health with the correct foods for your needs. All in All I can see why the high ratings and it will continue with me.

My experience at this facility has been very pleasant and efficient. I have found them to be very well arraigned and professional. The accommodations in facility are kept very clean, house keeping and kitchen staff are well managed, food is varied and well balanced and tasty. I have been also well entertained by the bingo they have here. Everyone is always so nice and attentive to me and I’ve been here for over a year already. Administration always making themselves available and are very responsive as well as professional.

I been to 5 different facilities and they don’t compare to this one, if I could give this facility 10 stars I would. The staff is professional, friendly , loving and family oriented. They provide multiple and different activities for the residents to choose from. The facility is very well maintained , clean , well equipped and professionally managed. Special shoutout to Emely , Christian , Silvia and Jessica for always being so attentive in my care and going above and beyond . The food service department offers a variety on their menu, my favorite is the baked potato. The therapy department is well staffed as well, I’ve seen progress in my life with taking therapy everyday.
